What crass and unnecessary rudeness!
Looking at viamichelin.com they do indeed look scarce - - and perhaps there are only these two (or three - - depends on your definition of "Eastern":
At Vysny Komarnik, between Tylawa (PL) and Krajna Polana (SK)
At Lysa n. Dunajcom, between Kiuszkowce (PL) and Velky Lipnik (SK)
and (furthest west - - serves the Zakopane area)
At Javorina, between Lysa Polana (PL) and Tatranska Kolina (SK).
Of course, this is "book learning" only - - but it's an interesting geography question.

We just made a crossing in both directions at Jakuszyce (PL), on the E65, between Turnov (CR) and Szklarska Poreba (PL), and found the entire experience pretty uneventful, taking about 3-5 minutes. It was strikingly less "inhabited" than most border crossings; essentially no nearby services (money exchange, gas other items for sale), and we never could find anyone who put a customs stamp on our tax-free documents (for purchases made in Czech Republic). We didn't worry about it too much since we were returning back into the Czech Republic right away, and we figured we would do better at the the Czech-Austrian border (between Cesky Krumlov and Freistadt), and indeed, we did later succeed.
