I'm treating my husband to 10 days in italy for our 10th anniversary next month! We want to see as much as possible in that time, so our itinerary is as follows:

2 nights Venice - 5 Star - Sant'Angelo Palazzo
2 nights just outside Florence at a farm/vineyard in Impruenta
2 nights at Casale Gregoriano a villa outside of San Gimignano
4 nights Rome - 5 Star - Hotel Bernini Bristol - right by the spanish steps.

We are using Delta FF miles and had no trouble getting tickets in business class. We fly into Venice and out of Rome. We will rent a car for the 4 days in Tuscany.

It's a fast pace, but it would seem to give the best of both worlds (countryside and city).

We took a trip for our 25th anniversary last fall - we flew to Rome, spent a few (not enough) days there, and then headed to the coast to catch a cruise on Windstar - a small, masted sailing vessel with only 140 passengers and 80 crew... very romantic.

We went to the island of Elba, Portofino, Monte Carlo, Nice, Marseille and disembarked at Barcelona where we spent a few more days. It was a great way to celebrate a wonderful occasion and to see a little bit of Europe. I highly recommend it! Neither of us speaks any other language except English, but we had a great time using "sign" language and the people we met were so helpful.

Of all the places we visited, I want to return to Rome and Portofino - have a great time, 10 years is a great milestone!
