We ate at the specialty dining restaurant on Millenium back in 2001. It was a fantastic experience and "top shelf." We were told the extra fee was part of the tip.
We just got back off Adventurer and ate in their specialty restaurant, and it was a huge disappointment, and certainly not worth the $40 that we paid. I didn't see anybody tip either time. You don't carry cash around on the ship -- just your cruise card which pays all the expenses.

We always carry cash when going to a specialty restaurant on any line and have always tipped based on the service. It is usually excellent and we tip accordingly.
On some lines we ONLY dine in the specialty restaurants every evening and we make sure the wait staff is taken care of.
