I would NEVER rent a car and drive it in Roatan (or any port!)! Not all the roads are paved and there are some very steep dirt roads! First time there we had a cab driver who had an accident going to Tabyanna with us and an accident when he was taking us back to the ship! Both were just little fender benders. Didn't bother him at all, never even got out of the cab with 2nd one! At the bottom of a steep, dirt road, he quietly said, "thank you Lord" - the brakes worked!
But other times we've had cabs with no problems at all. They know the roads and where they are going. Tell them you want to drive the 'low' road by the water. They ususally just drive you by the newer houses built by wealthier people. You'll see where the natives live on the water, we'd call them shacks but they are happy! They have a 2 foot dock going out and an outhouse at the end, the houses are built up on small tree trunks. We've seen school children on the steps doing homework and a mother in the water, with twin boys next to her, cleaning big fish. Some bridges are nothing more than tree trunks laid across the water. It is facinating and we always to come back to the ship on this route.
Best thing to do is get a cab to show you the island. Agree on what he'll charge you before you get in the cab. Then, don't pay him until you get back to the ship. We've gone to beaches and our cabbie just waited for us while we spent time on beach. Its a beautiful island, know you'll enjoy it! Let me knowif you have any questions and, .....Smile....Bonnie.J.............

Although some roads are unpaved, I found them to be fine. But I too would never rent a car there. I assume you mean have a private tour with a guide in his own car? I highly recommend Victor Bodden. http://www.boddentours.com/
He does not have his own computer and answers email via a web cafe, so be patient for a response. His english writing is not the best but has perfect speaking english. A very reputable vender to consider.
