Do you mean "direct" which in airline parlance means the plane you are on may make one or more intermediate stops before getting to your final destination or do you mean "non-stop' which is just that - there are no stops between your departure city and you final destination.
I'm not a SFO "expert" but I'm pretty sure there are no non-stops from there to anywhere in the Caribbean except perhaps destinations on the eastern coast of Central America. As for direct or connecting flights there are plenty. Depending on the airline and your intended destination you'll have to make stops at one of the major hubs, i.e. Houston for Continental, DFW for American, LAX or ATL for Delta, MIA for a few different airlines.
To help in your search the Caribbean destinations that can handle large jets that originate from one or more of the major US International Gateway airports include Jamaica, Grand Cayman, Dominican Republic, San Juan, St. Martin, Cancun, Cozemel, Barbados, St. Lucia, Aruba, St. Kitts, to name a few.

I also am guessing that Yipper really means to ask about a "nonstop" flight (not "direct" which can mean a stop or even a plane change but on the same airline).
As far as I know, no there aren't any from the west coast (for example, from Seattle to Jamaica the best route was thru Atlanta).

Sorry, no flights that I am aware of "direct to the Caribbean".
If you want to check such things yourself, probably the best place to try would be the Amadeus site (http://www.amadeus.net/), and the most likely destinations in the area from the west coast would be Miami (MIA) or San Juan (SJU).
I just peeked at mid-week flights, and there are no direct flights SFO-SJU or LAX-SJU, but there are direct flights from SFO to Miami (MIA) - and you can get to almost anywhere in the Caribbean from there.
Check on Amadeus for possibilities and fares, then make your booking.

Check other airports besides Miami (which does seem the most obvious). When I was looking from Seattle for Jamaica the Atlanta and TX connections were better options. My Jamaica flight on Delta thru Atlanta was really pretty easy for the distance (red-eye to Atlanta, then morning plane into Mobay).
