Went with GeoEx to Tibet/Nepal/Bhutan a couple of years ago. The group size was right (limited to 8 couples), our guide (we had a guide that stayed with us + guides from each country) and our accommodations both 5-star. I've heard nothing but good reviews about OATS from other travelers.
That said, unless you want to be with a group (we found it very limiting, even though the group was small) go with the advice to just plan your trip yourself and hire local guides. I just finished planning a February trip to Thailand, Laos, Cambodia and with the advice from all of the well-traveled and knowledgable Fodorites, it was easy.

Agree with the others. Although I haven't been to Vietnam, I have been to Cambodia (without a group tour). I think you can do it yourself if you want to. If you enjoy the company of a group and would like the convenience of having someone else take care of logistics, however, a group tour can be great. I've traveled with both G Adventures and Intrepid and enjoyed them. They limit group size and have great itineraries and reasonable prices. I will say, though, that we had more up close and personal interactions with locals when we were by ourselves. So I'd strongly suggest that even on a group tour, you find some time to go off on your own.
