I don't know of any company that runs tours out of Vietnam. It's kind of a closed system there, with all tourist activities essentially controled by the state-owned Viet-tours.
Your best be might be to check out Viator (http://www.affiliate.viator.com/broc...&AUID=1369). Viator is a sort of consolidator, offering day trips, side tours and other travel services. The services themselves are provided by local operators.

For hotels, I typically use www.asiahotels.com but there are many reputable hotel booking sites.
If you'll post your itinerary, I'm sure you'll get lots of comments and suggestions. I would book the flights in and out of Siem Reap ahead of time, as those flights are typically full.
