Most of the better hotels are outside the old city. It doesn't really matter because there's very little difference between "old city" and the rest of central Chiang Mai - the "old city" is not distinctly different.
In the old city:
Tamarind Village
DeNaga
outside, but central locations:
D2
Le Meridien (5 star but often affordable)
Yaang Come Village
There are many others, check a booking site like Agoda which has a map showing hotel locations.

I always use www.sawasdee.com to book hotels in Thailand. The prices generally include tax, service and breakfast.

WE stayed at DeNaga in late October and liked it very much. I booked it on one of the websites mentioned above (Agoda--I think). Like you I was a bit "up in the air" about where to stay in Chiang Mai--and saw someone's post on Fodor's about the DeNaga. Decided to stay there and glad we did. It is a nice hotel, great location and very quiet. Breakfasts were excellent and lots of great restaurants in the area.
