This is what I received from the company (based in India)who arranged our tour in India and Nepal:

Tips
For the guides : Rs. 300-500/day

For drivers : The driver who would be will be from Agra to Delhi : Rs. 200-300/day.

For driver who do the sightseeings in Delhi, Kathmandu, Varanasi, Khajuraho, the suggested tip would be Rs. 150-200/day.

For arr/dep transfers Rs. 50-100

Restaurants : 10% of bill if the restaurant isn't charging service charge.

Now that we are back...we had a question that we never found the answer to.
The front desk clerk always escorted us to our room and it seemed as though a tip was expected. We did not tip but did tip the bell boys (at least two came with our luggage!). The look on the clerk's face at one hotel indicated that a tip was expected.

We're going to India in November. I asked the tour company about how much to tip the driver and he said: "At the end of your trip (or each sector) it is customary to tip your driver. As a suggestion, 500-1000 rupees per day is a reasonable tip for the driver, but the amount is totally up to you." I thought this seemed like a lot. On the other hand, the Fodor's guidebook says 50-100 Rs per day for a driver which seems very low. I appreciate the comments on this forum in trying to figure this out.

@Erinwfs ,

Is your tour company including the service tax? I don't think there is anything customary about it. It's just courtesy i feel. And it should always depend upon your budget and how you have felt about the service. If you can afford 500-1000 rupees. Its fine but only after the trip is over not every sector.

Being a foreigner, people will think you are rich and may expect more. However do not go with their expectation and just do what you feel like. Tip should not be compulsion and pushed to customers. It should come just naturally. I tip often but when someone has really helped me and then i decided how much should I give. When you visit India , you will anyway get to know the value of Rupees and what is its worth. So while tipping just think what he could do with that tipping. May be gift for his child or one day meal or movie ticket. Enjoy!
