LHR to FCO is only 2 1/2 hr flight.
Not sure about Alitalia or BA but on LH intra-Europe flights business class simply means nobody sits in the middle seat. Same seats as coach, just the front of the cabin. A curtain separates business from coach.

BA also has rows with 3 seats where the one in the middle is the tiny one left unoccupied (Seats ABCs).
While not strictly all-business, many flights to Milan (or major business hubs in Europe like F'furt or Zurich) often have configurations with predominantly business class--2/3 of flights in business, the reat 1/3 coach. You should get simple but often pleasant meals in the BA biz class--hot breakfast for morning flights, substantial salad (eg salad with roasted chicken or salmon fillets on top) for the rest.

All flights from London City are close to this.
With TEN MINUTES checkin, six miles from The City and online checkin anyway, Air One to Milan (Linate, not that other airport practically in Geneva) and Rome is about as good as scheduled airlines can get.
BUT, they don't offer Singapore Airlines style pampering. Efficient, civilised service from a near-civilsed airport is the best you can get on intra-Europe flights.
Go to the City Airport site for other destinations. If pampering is important, click on the "Private jet" option.

I was on an airline once years ago, intra-Europe, and someone a row in front of me asked about how they were supposed to be in business class. Now, both he and I had the same sort of seats -- it was 3-3 and the flight wasn't that full, so hardly any middle seats were used, and those that were seemed to be traveling together.
But the stewardess checked something on a sheet of paper on, presumably on verifying that he was supposed to be in business class, moved the sliding curtain from in front of him, to behind him. Problem solved. Hee!
