If you are an American and you want to go to Cuba legally, you need to go to Cuba with a photographer who has a US Treasury license to travel to Cuba. Trips leave from Miami.
If you don't care if you are traveling illegally and are comfortable traveling from Canada or Mexico, then there are many trips available, including the one above.
There is a photographer in LA that goes fully licensed our of Miami, but is much more expensive than the link that was posted above by qwovadis http://www.lorneresnick.com/ National Geographic just announced 10 trips to Cuba in 2012 as well. Check National Geographic Adventures.
I am an enthusiast photographer who was in Cuba in 2003 and am going back early 2012. it is a wonderful country to photograph.
Of course, if you are not an American, it doesn't matter and you can go with anyone and travel from anywhere except the US.
Thank you Boston Harbor! Good to know about NG. I will check it out. Photogrphing the buildings and people during the day and listening to Cuban jazz at night sounds like a perfect trip to me!
