i plan to visit Bolivia from Atacama (and back to Atacama) through road on a 4 day tour in mid January 2026.
I am seeing conflicting information about visa requirements for US Citizen.
Can someone please confirm if i need a visa ? and if so, can i just get it upon arrival or do i need to apply for it online prior to the visit?

Also, do i need yellow fever vaccination ? what i understand is, since i am going from high altitude (Atacama and back to Atacama), i dont need the vaccine. A confirmation would be nice though.

A Yellow fever vaccination certificate IS required for entry into Bolivia. Though I have one, in many visits to Bolivia, I have never been asked to produce it. Whether you plan on staying at altitude or not is irrelevant as teh border police have no way of knowing. Personally, if you don't plan to go to the jungle ever, I wouldn't bother, if you do at some point it last for life. If you happen to be transmitting through Lima , you can get it at the airport clinic, very cheaply. Bear in mind it takes 10 days to become effective and for teh certification to be valid.

As far as Visas requirements are concerned wet from 1 Dec 25 , US citizens can enter visa free for up to 90 days. (check with the US state dept). Do make sure to get ALL the stamps and pieces of paper on entry and keep them for exit - officials at land borders are notorious for creating fictitious infringements in the hope of earning a "fee"

As for the yellow fewer vaccination: Even if it is not clear if you need it, but if you think of travelling more in your life then get it now. It will not getting cheaper and you won't getting jounger. And as this vaccination now is valiable for your whole live I would not hesitate.

Do, please, consult a physician before getting a yellow fever vaccination, and make your plans accordingly. Serious side effects can occur -- including death. The risk of deadly side effects is very low for young people, but is much greater for older people. And some people -- like those who are pregnant -- are generally advised against getting the vaccine under most circumstances. KJA is correct. Of course everyone should consult medical professional before getting vaccinated, either via their GP or a reliable travel clinic. They should not rely on anyone else. Repeating information given by a doctor is unwise as people often misinterpret what they are told, especially when the person repeating it, is translating into another language.
