Costa Rica: Currency & tippping

Good day-
We are headed to Costa Rica in the near future and I was wondering about currency. I've read that most places take US Dollars/ credit cards. From a tipping perspective: what is the appropriate percentage tip to give and should we transfer our USD to local currency or just use USD? your experience an input is welcome!

Restaurants usually include a 10% tip. If service is good, we leave a few extra dollars on the table. We've never exchanged our money on any of our trips. If you go to popular tourist areas, dollars are widely accepted. You'll get colones back in change. Some places won't take anything larger than a $20 though, and the bills have to be in good shape.
