I've found that some restaurants have restrooms in an area that is commonly shared with other businesses, and are accessed through a hallway. These types of toilets sometimes have a fee, since people not eating at the restuarant will also be using them, thus they are considered public.

I don't mind paying 0.50E to pee if the toilets are clean, but I have gone to plenty and paid for a nasty toilet. I've noticed that the McDonald's at Karlsplatz in Munich has an attendent with a plate for coins, but none of the local kids that I have seen contribute anything.
