Feel free to use a tour if you prefer, but do note that Greece -- and all the locations you mention -- are easy to visit on your own (although I personally wouldn't want to try cramming all of those locations into a single 12 day trip).

I agree with kja that this is easily done independently but agree that more importantly you are, I think, attempting too much in the time you have available. Greece, in particular, is one of those places where slowing down is more enjoyable.
I would probably choose between Crete and Mykonos/Rhodes/Santorini, and further I'd choose only two of Mykonos/Rhodes/Santorini. You could potentially fly to your furthest island and wind your way back to Athens by ferry. I'd stay in Athens last so you have a stress free departure back home.
I've only ever taken one tour (which probably isn't relevant to your needs) but I would always choose a small group tour (e.g. less than 12 people). The idea of travelling with 40 others fills me with horror but I know others don't feel the same. If you have a particular interest in archeology there are tours that specialise in that field.
