Here is an inexpensive hazmat suit. I guess you can buy it quantity if you are planning to do a lot of train travel.

You are more likely to be run over by someone luggage cart that catch scabies in the Milan train station.

How about Ebola or Bird Flu?

Perhaps those migrants will also be worried about catching something from you - I suggest putting bandages all over your arms and a sign: IN Quarantine on your front and back to keep them at bay.

I agree with nytraveler if you're ok at Penn Station you should be fine in Italy. Follow my rule, don't have sex with anyone in the train station.

I would not be as strict as welltraveledbrit.

Just use proper protection. There are vending machines in the train station's bathrooms. Have two one Euro coins handy and you will be safe. Enjoy your trip to Europe!

if machines are out of order carry some Saran Wrap that can also be used for those purposes and has been by many. Or a balloon.

In Italy she should be careful about eating anything in a restaurant with olive oil which could be adulterated by the mob - probably not harmful but you never know.

She should carry all her own foods for the whole stay.

IDH speaks of using a Hazmat Suit. I'm never without mine in all of my travels since 1946 and heaven knows how many horrible diseases I've fended off. I get sick thinking about it. They're on sale at COSTCO.

Have ONE carry-on sized roller bag and as noted above walk with confidence. Nothing says "Kick me" like tentative, hunched over posture and a dazed look, accompanied by numerous parcels.

Even if you might be going in the wrong direction, do so with conviction. You can always walk back.

BTW I have been often to Italy and often alone, including Milano Centrale.

The ONLY time I had trouble was when my hubby was 10 feet away (Brussels train station area).

Carry hand sanitizer and have a wonderful time. BTW Hotel Berna is a nice hotel not far from the station.

I don't do carry-on when I'm travelling overseas, but I have a small/medium sized bag and a cabin bag that sits in top of it very easily. It's a convenient unit that I can manage efficiently, up & down stairs & hoisting up onto luggage racks in trains if necessary.

And I agree - nothing says "target " like someone standing amongst a pile of luggage looking lost.

Caveat: scabies may be less vigilant than pickpockets for this invitation to get up close & personal.
