Amalfi Coast and what other country
We will be staying in either Positano or Amalfi for five days and 4 night and was thinking for the remianing 4 nights to go to another country as we have already visited Rome, Florence and Venice, Is there another country that is accessible from Naples that someone can recommend with the exception of London?

Well I spent 10 days on the Amalfi coast a few years back. It was GREAT! Spent a couple of nights in Naples mainly to see the Arch museum and the veiled Christ. Then went to Pompeii and Sorrento for the day. On to Capri for about three nights and then to Ravello for the remainder of the time. This was not too much time for us., but I realize we are all different. Sicily is an option though I have never been there. (It is on my list) Friends went there after their Amalfi coast trip traveling on south and crossing by ferry. There is also the south of France. I would certainly check travel connections as you could spend most of your time getting somewhere. Happy travels!!!

You've only touched the surface of all the wonderful things Italy has to offer so I would advise staying in Italy and seeing more of that country.
If you have to leave Italy you could always fly to Spain, France, or Greece but 4 days won't give you much time any place.
PS London is a city, not a country.

I really liked Sicily in the early Spring specifically, Palermo, Siracusa, and Agrigento, but if you want different country if you can find a convenient flight out of Rome then my pick would be Croatia, specifically the Dubrovnik, Kotor Split area in the Balkans, when I arrived there I liked it so much I stayed a couple extra days.

We adore Sicily and had the most wonderful trip there. However, I don't think you can really do it justice in 4 days. What time of year is your trip planned? Have you been to Tuscany or Umbria? What about Capri, Naples and Pompeii which are close to where you will be. Or the Lake country of Italy. There is so much to see and do in Italy that I wouldn't search for another country for just 4 days.

Four days is no where near enough for Sicily. I would base yourself in Sorrento and visit Pompeii, Herculaneum, and Naples, especially for the archeological museum. You could add a day trip to Capri or Ischia for the forth day.
Of those places only Naples is a city. If you definitely have to be in a city I would vote for Palermo for four days.
Where are you flying home from?

Although it's definitely not entirely correct, and includes routes that are only seasonal, you might start with the "who flies where" list offered by the airport in Naples?

Milan and Rome are only choices if flying from the only other one nearby - just outside Salerno...

Sept and Oct flight times here...

You can easily fill another 4 days in Italy in that area. However, if you'd really like a change of scenery and country, sometimes there are good travel deals flying out of Naples. I've flown to/from Paris, Madrid and Prague, for example, and next up is Brussels. Not long flights and if schedule works right you don't lose an entire day traveling either.

Another site just beyond the AC near Salerno is Paestum, with some of the world's best preserved Greek temples; it's a World Heritage site. And then there's Herculaneum near Pompeii. And Mt. Vesuvius, and the island of Ischia as well as Capri.
Also some of us return to Rome or Florence or Venice many times -- to see things we missed previously. It's hard to believe you've exhausted the sights in Rome, for example.
