Mark,
Many of the local restaurants are very good. Try the one under the arches opposite the west side of St Nicholas Church near the top end of Mostecka. Plain surroundings, great food, violinist and really friendly service. At the top end of the street there is a covered lane (a 'pend' if you're a Scot) that leads onto Trziste and from there a short uphill journey to the Constans. If you are looking for something a bit more upmarket you could try the 'U Patrona' in Drazickeho namesti. This square is just off to the right as you leave the Charles Bridge. Again, one I could recommend personally.
Can't recommend a personal guide as we didn't use one, but I would suggest you try three of the very short tours available. Each lasts about 2 hours.
a)The tour of the local pubs (3 pubs, 3half litres of Czech beer, and good company)
b) The Jewish Quarter
c) The tour of the Old Town Hall-NOT just the visit to the top of the tower.

The last one can be booked at the tourist office beneath and to the left of the Astronomical Clock. Not only do you visit the top of the tower, but the lower rooms, and the basements and underbuildings.

Can't speak for hotels but daily fully furnished apartments are a great way to go with extra room, some with balconies and cheaper than hotels. We have been to Prague three times and this is the ONLY way to go. I prefer being closer to Old Town but either way, you'll have a great experience. Have a marvelous vacation. Cindy

Mark,

I stayed at the Hotel Metamorphis in January for my first trip to Prague.

I LOVED the hotel...Very big rooms, clean, with a view of the Tyn Cathedral (make sure you ASK for a room with a view when you book). It was very centrally located, and honestly, I don't remember it being noisly at all. Mala Strana (Little Quarter) is a nice area too, but not as central as Old Town...

I would absolutely stay there again.
