Northern Italy
We've been to Italy several times but never to Lake Como, Cinque Terre or the Italian Riviera.
We are retired and in good shape. When is the best time to go? We would like to spend 11-12 nites In northern Italy. What else are musts besides the above? We plan to rent a car. Should we fly into Milan, Venice or another city to start the trip?

It depends on whether you want to see Venice. For the places you list you want to be on the North west side , so Milan is the best airport of any size. You may however, be able to fly direct into Genoa or Pisa, both of which put you in the right place. Check on where you can fly to from wherever your local airport is.
What time of the year do you plan to travel? Time of the year will dictate how busy various places are. For me, spring or late Summer/early Autumn are the best times, once the high season has past or yet to come.
Try Liguria as a great place to visit and Cinque Terra if you must and want to do a lot of walking. Not my favourite place at all for which I blame Rick Steves.

When you say "best" times, are you asking about weather, crowds, bargains, other? If your answer is "all of the above," then I don't have an answer.
As far as other "musts": With 11-12 nights and three destinations already planned, you probably don't have time for other places.
You don't list Venice as one of your stops, so I'm not sure why you would consider it as your landing point. It seems as if Milan is more practical, although Pisa may be an option, as the poster above mentions.

It's not necessary to rent a car for this trip. You probably don't need it for Lake Como and definitely don't want it in the Cinque Terre. (The 5 towns are linked by train, boat, and footpath. The road is back inland.)
You can stay elsewhere nearby on the Italian Riviera and make a day trip to the Cinque Terre. Note that the trail between the CT towns suffers from winter storms so may not be repaired in earlier spring.
