I don't know Monsummano, but have thoroughly enjoyed Bagno Vignoni, San Casciano dei Bagni, Rapolano Terme, and Chianciano Terme, which range from rustic to opulent. Be advised that for some of the treatments you need a doctor's prescription. No problem if you just want to "take the waters," though.

From Genoa you have really no choice but to take a train as far as Firenze, at which point you can either take another train to Siena or a bus. Depending on where you are staying, the bus may be the better option, since the bus leaves you in the center of town while the train station is a bit outside of town.
From Rome, you can either take a train to Firenze or a bus.
If you will not have a car at any point during your stay in Siena, and you are looking for a spa, only Rapolano Terme is easily accessed from Siena by public transportation (train) for a day trip. Otherwise, if the point of the spa treatment is relaxation, you might want to overnight if you make the complicated trek by public transportation to Montsummano Terme or other places mentioned.
