It is usually better to make advance reservations before your trip, but you can also make reservations once you arrive. I suppose it would depend on the time of year you are going and how booked up they might be.
The tour is about an hour and a half from what I recall. After that, you are free to explore the rest of the palace on your own.
You can get all the info you need, including the number to call for reservations, on the "Helpful Information Italy" thread. You can do a text search above for it.

I do not recall the exact cost. But depending on the season, you do not need reservations. We were there in late May, purchased the tickets and happen to get the tour in English ten minutes later. The tour itself is a couple of hours at the most, but you will want to visit the rest of the palace afterwards, so count on 3 or 4 hours for the entire palace, at a minimum.
