I stayed at the following place while in Vienna. And since it's clean, very centrally located (yet quiet), near transportation and moderately priced, it definitely meets your criteria. The hotel's information and my comments are as follows:

Pension Pertschy
Habsburgergasse 5
Vienna, Austria A-1010
011-43-1-534-49 phone
011-43-1-534-49-49 fax
This place was recommended to me by someone who goes to Vienna frequently. This hotel was perfect for me and so I have added this to my list of places that I would definitely return to. I had a Single which was very large with my own little hallway containing a table/desk and coat/hat rack. The bathroom was off of this and upon turning a corner I had a separate room that served as my bedroom. The bedroom was very large with a double bed, tv, minibar, telephone and a wardrobe. I found it to be very quiet (I was facing into the courtyard) and had to remember to keep my curtains closed since my windows (floor length) opened onto the passageway that went around the courtyard. A wonderful buffet breakfast was included in the price. Note: The hotel has an elevator and stairs.

Since I haven't yet been to Berlin and Prague I can't offer any suggestions. But I'm sure others on this site will be able to come up with some suggestions for you.

A site that will help you in all 3 cities is nethotels.com.
In Vienna, I have 2 recommendations. Hotelpension Arenberg (4-star) is within the Ring, 2-3 minutes to U-Bahn, 3-5 to Air Terminal (where buses from airport arrive) and 5-7 minutes from Stephensplatz. It is in a nice area, part of Best Western but with only 24 rooms retains that posh Vienna feel. You have bathtub as well as shower & toilet, homemade muslei for breakfast. The other is Hotel Drei Kronen (3-star) and 3-5 minutes to U-Bahn, 6-8 minutes to Opera, has nice buffet breakfast, near the University, shower & toilet in room, nice open-air market across the street and grocery in next block.
In Prague, we stayed at Pension Balbin on Balbinova, near the Muzeum metro & Wenceslas Square. It is on the top floors of an apartment building with locked door to outside so safe, clean, shower & toilet in room, 5-10 minute walk to just about anywhere you want to go.
Can't help with Berlin as stayed with friends.

We also stayed at the Balbiln in Prague. Were very happy with it. In Berlin we stayed at the Hotel Funk. Walking distance from the train station and just one block off the Kudam. It was once the mansion of a silent movie star in the 20's. Very comfortable rooms and lovely manager

In Berlin it was Hotel-Pension Funk

Hotel-Pension Funk
Fasanenstraße 69, 10719 Berlin - Charlottenburg
| Tel: 882 71 93, Fax: 883 33 29
| U15, U1 Uhlandstr.

A cosy pension in an apartment which used to belong to the Danish silent movie star Asta Nielson. The rooms are comfortable and have their own telephone, and are furnished in the style of the 1920s/1930s.

I stayed there in 1998 and it was one of the highlights of the Berlin trip.
