I'd like to second the recommendation for Hillsandroads.com. The owner, Luca Garapa, is a delightful tour guide, and he will customize an itinerary to your personal tastes. We spent a few days with him last summer in Chianti, and it was a real highlight of our trip! On top of that, he was the most reasonably priced of several driver/tour guides I contacted.
