There are innumerable ferries, hydrofoils, catamarans and excursions boats operating around the Eolian islands. There are also various links to the other seven islands around Sicily. I don't know of any cruise ships based in Sicily, but some of the ferries to and from the mainland are like cruise ships - try Grandi Navi Veloci (www.gnv.it) who run to Palermo from Genova and Civitavecchia.

These are not "cruises", but rather local transportation. Fares are low, and service is pretty reliable, except during bad weather (mainly in winter), when service is sometimes cancelled.
You can also take a ferry ship to Ustica Island (from Palermo), the 3 Egadi Islands (from Trapani), Pantelleria Island (from Trapani or Mazara del Vallo) and the 2 Pelagie Islands (from Porto Empedocle). The ferries are not luxurious by any means, but are great fun and, in sunny weather, afford magnificent views and videos/photos.
