You usually are better off going through a broker such as www.kemwel.com, if they have an agreement with an agency in Civitavecchia.
I've also used this broker in Italy: http://www.economycarrentals.com/ but the prices are very competitive only the car is returned to the same location.

I usually use www.izzirent.com . I like them because they compare rates from almost all global brands and local too.
