Not by train - 15-20 hours and requiring use of overnight train from Medina del Campo - check out bus, probably quicker but maybe not - long way through sparsely settled places meaning lack of good public transportation. check on flying.

Bilbao is not the easiest place to get to/from by rail. It's the terminus for FEVE; but that won't get you to Porto.

As I recall, there was an overnight train from San Sebastian to Lisbon via Porto. Check http://www.cp.pt (the Caminho Portuguesa website). Click on Internacional.

You may have to backtrack to San Sebastian to pick up a train into Portugal.

You may also want to check out Portugalia Airlines http://www.PGA.pt
They may fly that route

jackie_o, a few years ago we tried to do that. Although we took a train to Bilbao from Barcelona, we rented a car in Bilbao which we eventually dropped off in Madrid. After tons of research we decided we just couldn't do northern Spain and Portugal by public transportation.
