No specific nightlife recommendation, but my general experience travelling is that, if you want to find nightlife that suits your age/type of entertainment/atmosphere preference, you should ask some of the local people working at the hotel where the hot spots are. Be specific re: what you are looking for. For the most part, I have gotten really good advice this way.
Re: restaurants -- there's a seafood restaurant called Cobia in the Condado Plaza Hotel that I really like. There are also a bunch of funky restaurants in Old San Juan (they're mostly on one street, but I can't remember the name). There's a nice lunch spot in the atrium at the El Convento Hotel in OSJ. For cheap local food, try Metropole, which I think is in Isla Verde -- it's not far from the Embassy Suites Hotel.

For an inexpensive full breakfast ($3) Check out Mi Casita. It is a small restaurant in a strip mall across the street from the Intercontinental Hotel. It is opposite a Church's Chicken franchise restaurant. They also serve reaonably priced lunches and dinners. They feature Puerto Rican dishes for lunch and dinner. Breakfast is typical American - eggs, bacon, ham. potatoes. Another popular restaurasnt in Isla Verde is the Metropole.
The dance club in the El San Juan hotel is considered to be one of the best in San Juan.
Metropole is a good choice for P.R/Cuban food at a very reasonable price. Sonny's, which is an open air restaurant right on the water is good also. Any of the restaurants in the El San Juan are excellent, although pricey.

For nightlife - El San Juan is an excellent choice. Very big lobby bar, live music, and also a nighclub (called Babylon) that gets packed. Plays a mix of Latin and American dance music.

Just as any fyi - at night, people tend to dress up in P.R. - no shorts and t-shirts.
