Many of the islands (Nevis, for example has a 6 month quarantine, so you would not be able to take your dog anywhere), have very stringent regulations on bringing in dogs because they have no rabies.
Some are not so strict.
One issue I could see is that MANY islands have a large stray dog population, and I have seen a few vicious ones (not many). They tend to be large and it would not be good if one were to go after your small dog.
Many of the islands have at least a few (in the case of some islands, LOTS) stray dogs roaming some of the beaches.
Vaccinations are not common on most islands, nor is modern vet care/facilities.
The ONLY way I would consider taking my dog on vacation with me would be to stay at a private villa.

Dogs are allowed on all of the beaches in Anguilla & there's no quarantine. Here's a short version of the requirements: To get an Import Permit form -- Agriculture Dep't.
