If you're planning to have your car with you all week, it's best to pick it up from the airport and just drive yourself to Sapphire. What's the point of going to Sapphire and transfering somewhere else to get your car, right? If you check the prices, they're all about the same anyways, especially in January which is high season. Hertz is good, but I have found Budget to be most competitive in pricing.

We stayed at Sapphire Beach (Crystal Cove) and rented for the week from Avis, which happened to have the lowest rate at that time.
A couple of things to consider:
Some rental agencies will allow you to take the car on the ferry to St. John; others won't.
There are a few rental firms that are not at the airport and thereby avoid the airport fee - might save you a few bucks (I think Dependable is one of them).
