My advice is no matter what the additional cost, get an automatic. The left driving is a little bit confusing at first. But in while you're adapting, the last thing you want to be doing is having to grab for the stick when it's not in the place you expected.

The toughest adjustment seemed to be for turns...especially right hand turns from a one way street..in traffic( U- turns can be a little disorienting at first). It helps to consciously think.."keep left" or your instincts may take you to the wrong side of the road.

We only reserved an automatic car as our first rental...so we only had to get used to the left side driving. After that, we rented manual transmission. At first, I drove...and my wife shifted when i told her "shift"...we got synchronized pretty well...after that it was no ral problem.
I would also agree that turning at intersections was the hardest thing to keep straight. It's amazing how you don't think about it until you have to!
I drove in Grand Cayman recently, and used a left hand drive for the first time. It made everything a lot easier.
The only problem I had was every time I wanted to use my turn signal, I ended up turning on the windshield wipers!
