Packing list for Myanmar

Packing list for Myanmar
We are going to Myanmar in early January, so just wanted to start preparing. Please help me out with my packing list. Here is my list with some questions. I would appreciate any additional suggestions:
1. Hiking shoes (we have day treks - 2-4 hours. Not sure if we need boots
2. Slip on gym shoes for visiting temples, so easy to remove. Some people suggest even flip flops, but not sure how comfortable
3. Sport sandals (not sure if needed)
4. I heard we need to bring dollars in mint condition. Is this still the case or we can get local currency in ATM? Do we really need to have dollars on hand, or just for an exchange to local currency?
5. We will have an international plan on our ATT serviced, but not sure it will work everywhere. Is there a better option?
6. We already got a letter of approval for visa (e-visa). Do we need any additional photo with us for any reason? I was reading we need a photo just to buy s SIM card.
7. Antibacterial wipes
8. Prescription meds, and something for stomach issues
9. Light Jacket for Inle travel (we have long travel from Inle to Loikaw)
10. Several pairs of long light pants, shirt/t-shirt with sleeves, sweatshirt
10. Hat, sunscreen, mosquito repellent

As our trip included NZ in winter thermals, gloves and hats were needed but I am guessing would not be a pre requisite for Myanmar, so that would cut down weight and volume considerably.
Hiking around Asia we have always used trekking Sandals, such as Keens, rather than boots which, generally are only necessary if you have any ankle issues. Otherwise, too bulky, too hot and not exactly packable.
Flip flops - for year these were anathema to me. I borrowed my wife’s pair recently out of necessity one day and realised th error of my ways and have just ordered my very own pair from Crocs..
Instead of bacterial wipes, take a small bottle of hand sanitiser. Lasts much longer and more versatile.

Comments: You are packing too much! Especially in terms of footwear, You will not need hiking bots, sports sandals r similar will be fine. I wouldn't take slip on gym shoes - they will get wet and uncomfortable. Take a really good pair of flip-flops that offer good support. Foot wear takes up a lot of luggage space and they are heavy. I always use flip flops for going in and out of temples.
Myanmar now has ATMs that allow you to access local currency. It doesn't hurt to take a few pristine bills along for exchange as needed.

I was last in Myanmar May 2016, so things might have changed. No photo ID was needed to buy a SIM card. I got an Oroodoo one for 5000 kyat, top ups are available everywhere. The E visa worked really well, no extra photo was required.
Agree with others about footwear, boots not necessary and flip flops are my footwear of choice for much of SE Asia. ATMs are everywhere and dispense Kyat. You might still need pristine USD for entrance fees to certain places, Inle Lake being one.

Take a fleece rather than a sweatshirt==much faster drying.
Our feet were filthier in Myanmar than anywhere else in Asia we've visited because of the fact you can't even use "temple socks" if you'd want to. Take the wipes.

I noted on money, SIM card, photos, shoes, and fleece.
As for hiking shoes, I was planning on bringing a pair of short hiking shoes like Merrell (not boots), as I had ankle issues in the past (broken once, and badly sprained multiple times), so I thought they would offer a better protection.
