You request is so general, there is no way we can tell you where to go. You need to start by doing your own research. Get a good guidebook on India, read some of the India reports here and see what appeals to you. There is so much to see and do in India! Once you have narrowed down where you want to go, we can be more helpful.

Some of the top places in India where you can enjoy adventure activities and adrenaline rush with some relaxation are:
Goa and Uttarakhand
-Goa is a beautiful and lively holiday destination where you can enjoy water sports. Some of the beaches are superb. Don't miss Fort Aguada. Sea view from this place is just amazing!

-In Uttarakhand, places like Naukuchiatal and Mukteshwar are nice as far as kayaking, paragliding, rock climbing, Zip-line are concerned. Kayaking near The Camphor Tree - Pura Stays is a good option. Whereas, if you want to just relax and have awesome panoramic views of Himalayas, then Frozen Woods is another option. Visit Ranikhet if you wish to. It is near Mukteshwar and is well-known for offering peace amidst nature.

Have you done research with guide book and maps. They are the ones that will give you a wonderful idea where you want to travel, how far you want to go.

Thanks for those suggestions, @anki_123456!

I have been reading online articles lately about cities and found out that Delhi is running towards the top for the most polluted cities in the world (according to the article). How true is this?

Nice plan to travel in India during Nov month because Dec and Jan Months High session, you have lot of time first half month spend in North India(I think already done), second half month you will spend as per relaxation in backwater, with Yoga best place in Kerala, South India.
