Looking for a winter retreat
Our last granddaughter graduates this coming year & we will no longer have sports to attend so looking towards January & February 2026 to find a tropical destination that is easy to get around without having to rent a car the entire stay. We are mid 70's, active & in good shape, love to snorkel, visit ruins & places with historical sites. We ride bikes whenever possible. We have traveled extensively including Peru, Africa, Columbia, Guatamala, Costa Rica, much of Mexico (loved Holbox) & French Polynesia. If you had to pick a place to stay at least a month, near water, where English is common where would you choose?

Totally Recommend Belize!
Looks like you've missed Belize in your Central America travels. The official language of Belize is English, so where ever you go in Belize, English is spoken.

We have been to Belize (Caye Caulker & San Pedro). We considered Placencia but appears to snorkel you need to do a spendy tour. Love to see ruins (Tikal is on my list) so San Pedro would work.

Not sure how the snorkeling is around Bacalar Lagoon but the swimming there is great. Ruins and nature at Calakmul are excellent. We spent a few nights at Chicanna Ecovillage when visiting that region.

Hawiian islands are absolutely incredible. But you need a car to get around. There are no ruins. Bikes would not be practical for many locations. You can find snorkeling and (depending which island) historic sites. English is widely spoken.

Here's the part of the question I think has to be addressed:
I enjoy each one of the places mentioned. Many of them, I couldn't see spending an entire month there. Remember: Spending one month in a destination will be a lot different than just a few days there. It isn't just snorkeling and ruins you need for a stay that long. Where will you find a short-term rental? Where will you eat? Where will you shop for groceries and other items? Where would you go for medical attention if needed? (Let's hope that doesn't come to pass.) Of the places mentioned, I think Puerto Vallarta offers the best of that type of infrastructure. It's warm and tropical and resort-y, but developed enough to have a good variety of what you need for a monthlong stay. I trust suze as the Fodor's Forum's "Ms. PV," who has stayed there long-term, to be able to look at it from that perspective.
For me, that would open up places like Oaxaca or Merida, but they don't sound like what the OP is looking for. Well, ruins nearby, yes.

We do love Hawaii & would think it perfect for a long stay but it's pretty spendy. We will be spending 18 days there in March (one week on the Big Island, one on Kauai & 3 nights on Oahu). We've been going there since 1995 & have a timeshare at Lawai Beach on Kauai. We can walk across the street & snorkel there from January on as long as there aren't storms. We use to dive but have settled on snorkeling now. On the Big Island we have done the night snorkel with the manta rays & explored the VNP & most waterfalls. Oahu is my least favorite due to it being so busy & big. We have been to Puerto Vallarta 3 times & had the best zip lining. We stopped there on our Panama Canal cruise this year & it felt too crowded. One of the places we haven't been is Panama so curious about it. Appreciate all your suggestions

We have been to Turks & Caicos, Punta Gorda, St John & loved them. The water was beautiful at St John & Punta Gorda. We spent a week on the Caymans & must say we hated it. We wanted to dive but the coral was ruined & the water murky. Snorkeling wasn't great either & what appeared to be sand was sharp coral bits.
We have enjoyed Akumal, Holbox, Cozumel & the Bay Islands of Honduras. We really seem to enjoy low key places that aren't too crowded, relaxed vibe, cute little beach houses or thatched roof bure like in Fiji. It's hard to find a place where we wouldn't have to rent a car for a month but I'm hoping we can. I haven't ruled out Costa Rica however just read an article that said that after Covid & all the remote workers that prices have skyrocketed & locals can no longer afford to live in beach towns. Love all the activities that CR offers. We've ziplined, rafted the Paquare River & the Sarapaqui, soaked in hot springs, etc. Appreciate all of your suggestions since I know you are well traveled.

Panama's public healthcare system is less expensive than stateside, but it's not free. The parallel private system is very good, but more expensive than the government system (but less expensive than back home). You'd definitely want insurance that would cover you during your stay.

I think it was 2011 when I was in Panama, and at that time I think they offered $7000 in free emergency medical care for tourists. I didn't know if that was still the case. I did a little googling and it looks like the program was canceled in 2017 due to fraud (shocking!). But it looks like in 2025 the program will begin again. According to this article, which isn't an official Panamanian govt website. Seguro al turista was what it was called back in the day.
