Single parent signature

I am about to buy airline tickets to fly to Costa Rica with my wife, daughter (16) and her friend (16) for a summer vacation this coming August.

I know that my daughter's friend needs to have a Parental Consent Form signed by her parents.

My question is, my daughter's friend is raised by her mom. Her father is not to living with them and no where to be found. Is her mother signature on the form is enough?

the legal custodian needs to sign, you probably would be wise to carry the papers that prove legal custodian of the child also.

I'd also get a medical release signed, so there wont' be any issues if she becomes ill or injured.

I'd have the papers notarized as well.

Supposedly, you are required to have a signed consent form. This has been the law for as long as I can remember.

I have single parents traveling all the time, with no issues.

When our daughters were teens, we would travel when them all the time. The funny thing is that my girls are actually my step children, and use their birth father's name. My wife uses her maiden name, and of course my last name is different. So our family group had three different last names. We were never questioned. Not even once.

But our youngest is 30 now. So it has been awhile.
