You could find numerous options in each of your three options. I am very partial to Montana, especially the Missoula area, you got Rock Creek, Bitteroot, Blackfoot, Clark Fork, etc. I would google fly shops/guides in an area you want and you'll get plenty of options. I wet my line in each of the areas and have never stayed in a lodge, but you can't go wrong with your options.

Like Brando I've never stayed at a Lodge nor used a guide however I've hunted and fished along the Madison River from Hebgen Lake to Ennis. http://www.gonomad.com/destinations/...a_fishing.html
