There is a drive-in theatre in Tafton PA on Rte 6...Maple Drive In. Awesome popcorn. This area of the Poconos is very nice-right on Lake Wallenpaupuck.

Transit Drive In on Transit Drive (Route 78) in Lockport, NY (so named because it is home to some of the Locks of the Erie Canal) of which is near buffalo, NY and Niagara Falls, NY. 4 screens, miniature golf and playground, all sorts of food, celebrating it's 50th year in business. You can tie it into a trip to Buffalo/Niagara Falls area, then head to Finger Lakes or Toronto for a a really nice time.

There's one on U.S. 68 between Kenton and Findlay, Ohio. I don't remember the name. I've only driven past it, but it looks like very little has changed since the '50s.
I also think there's one in Springfield, Ohio on U.S. 40, but I'm not sure about that one.
