We will be on a cruise of the islands in mid May. How do we get a rental car from the ports instead of the airport?

Go on line to the rental car companies direct and plug in the area instead of the airport location. They'll come back and tell you if there is another location available besides the airport location. ((b))

Since you will no doubt land at Aloha Tower when you get to Oahu, this is the best resource for you anyway.

On Maui Kahului Harbor is next to the airport. I have seen the tour busses leave the port on boat day and head over towards the airport. I assume they are going to the rental lots. If your boat goes into Lahaina, there are rental companies in Lahaina.
