There's a Garlic Rose in Madison, NJ. It gets mixed reviews. Cramped quarters, OK food, BYOB.
There's a garlic festival in Gilroy, California every year, featuring many garlic dishes, including everyone's fave, garlic ice cream. Not kidding.
There is one called NinnikuYa in Honolulu, on Waialae Avenue. A little pricey, but good. (Ninniku means garlic in Japanese)
The Ark, located in Nahcotta on Willapa Bay in southwest Washington State, has a garlic festival featuring chocolate covered garlic. Great if remote place.

Gilroy, CA calls itself the Garlic Capital of America, and when the garlic is high and the sun is warm, you can really smell it in the air--heaven! The festival is a blast, actually, if it's not a thousand degrees out--great food, including the garlic ice cream (nowhere near as weird as it sounds), some arts and crafts, and if you're lucky, you'll see the Garlic Queen and her princesses, all dressed as garlic bulbs. Cute.
