We are planning on a 3 day stay in Charleston next week. We will be going out on the water. Our question is should we just do the boat cruise or just the ferry to Fort Sumpter or should we do both trip? What would we miss something if we did just the cruise or the ferry?

Definitely go to Fort Sumter. I've included the link to the authorized concessioner of the ferry which takes people over to the Fort Sumter National Monument. After browsing through what it includes, you'll be able to see if the other boat tour you're considering has any overlap.
