The subway is quite easy to use. Almost every line runs north/south in Manhattan. The 4, 5,6 run on the east side. The 1,2,3 A, C, E, run on the westside and the B, D, F, R, B, Q run up the middle of the island for the most part.
There are signs that will say Downtown and Brooklyn or Uptown and The Bronx, or Uptown and Queens. The most confusing thing for a first timer is understanding which trains are express and which are locals. The best thing is to ask.
By the second day you will be a pro.

The MTA official site of NYC's transport system has a trip planner that will route subways trip for you, as does Google and Hopstop.
