Manual Antonio has more tourist, but of course that is for a reason. The national park right there along some beautiful beaches with nice wide paths and lots of wildlife made more evident by excellent guides. . .more activities of course, to entertain tourists--Finca Valmy offers an excellent horseback ride to a waterfall, mangrove tours, etc. LOTS of hotel and restaurant choices.
Malpais is a surfer's paradise. Used to be very laid back, off the radar, but not so much anymore. Still less traveled than MA. It, too, has a nice beach--just different. Nearby is Cabo Blanco Reserve where you can do a very strenuous (take lots of water) hike and see plenty of wildlife on your own. Montezuma town is close as well, and Montezuma Falls a fun visit for a swim at the base. There is less to do in and around Malpais. You could do either.
We visited Costa Rica several times before we ever made it to Manuel Antonio. All of the areas are so interesting and unique, it's very hard to make a wrong decision. Malpais is more difficult (takes more time) to get to if that is a consideration.
