Stephen:
I just returned from a trip to Italy and
began in Milan at the Malpensa Airport.
The Malpensa Express is a new and very
fast and clean train that runs from the
airport to Milan. You can ride for no
charge if a passengar on Italia or Continental. Need to get a taxi to the
Stazione Central or do the subway. I
suggest a taxi as it is much easier and
not expensive. These trains run all of the time and there is a place in the
arrival terminal to get information. I found out about the Express on the Fodors Forum or the website for the Malpensa Airport or AOL
Travel Board. Can't recall which.
Good luck.

Definitely the bus. It leaves every half hour, and you can buy the ticket from the driver as you get on the bus. I would give yourself a little extra time and not cut it too close, as our bus didn't leave exactly on time, and traffic can be unpredictable.

Hi
I did the same trip last October. I agree with Cheryl to allow a lot of extra time between your plane arrival and your train departure. Last fall
the bus from the airport to the train station took 90 minutes to make the trip in the early morning--rush hour, I presume. I will also add that it is best to travel light. Lugging heavy suitcases off the bus, through the station, onto the train, through the car, onto the luggage rack, and off the train, taught me a very valuable lesson about packing light.
