In most of the places where I've been on public transportation, there are always people who will give their seat to someone with a small child. The ride wasn't hair-raising at all; although it does go up and down hills, the speed was very sedate.
This tram is written up in all the guide books as having a scenic route, which is why it's so packed. I thought it was a bit oversold, but we were on it when it was already getting dark. Half the seats were empty at that time of day. I would think you could avoid the crowds by going at any time after 5 or 6 PM. We got on at around 7 PM, but it got dark earlier when we were there.
I wouldn't take a pram on this tram, though, because, as others have said, it's very small. A folding stroller should be fine.

I would not advise trying to take a stroller (or two!) on the old fashioned eléctricos. They are narrow with no space to park the stroller. And they will be extremely crowded, like a can of sardinhas.
If the three year old is into trains and trams, this will be quite a thrill, and I think it would be better for one parent to take him for a ride and meet up with the rest of the family later.

Thanks everyone, i think we should not stay in Alfama and forget about taking the tram 28. With small kids and a folding stroller, it will be way too much of hassle. It seems we should stay somewhere in Baixa/ Rossio to be able to enjoy the city more with kids.
