Only in Lazio (Rome) will sending the bread away save you from the charge. Everywhere else, the charge is for "pane e coperto", and you'll have to pay it whether you send the bread away or not.

Pizza is not only for tourists; Italians eat it as well. "Serious" pizzerias in Rome only serve it in the evening, but the pizza al taglio (pizza by the slice) places have pizza, a very different kind from the Roman thin-crust pizza, at lunch. It's for take out, and it's eaten with the hands.
