For the part in Slovenia it looks a lot like what we just did.

don't you go to Ljubljana ?
We also went to Trieste. Quite nice.
We didn't go east.

Have fun.

I don't know about making reservations so much in advance.
When in Bled we had difiiculty to find a room in june, something like 15 days in advance.

Look at booking.com with your dates, the site will tell you if the period is under high demand.
It is a little bit biased since it gives only the demand for the part allocated to booking.com but it should give an idea still.

Have fun, the region is gorgeous for hikes/walks.
We awalked along a river - forgot where - on a well maintained path, very nice.

Hi

Ljubljana is small and very green: Is not like other bigger town. I highly suggest you to visit Ljubljana.
Mozirski gaj in that time of the year is not exactly the best option. Flowers are mostly gone. It's autumm.
Add Jezersko. From Logarska dolina go to Austria to Pavlic pass and back in Slovenia. Very scenic route.

Maybe add Goriska brda near Gorica (south from Kobarid). Wine region with nice colors in that time of the year.
Near Cerknica visit Rakov Škocjan

Some time ago we spent roughly 2 months living and travelling around Slovenia & Croatia. It was a lovely experience, so here are my tips and personal favourites.

I will highly recommend to visit in Slovenia the Bled lake, Postojna cave (Postojnska jama in Slovenian), Predjamski castle (Predjamski grad in Slovenian) and Koper.
Postojna cave is a beautiful with vast network of branches. It even has its own mini-railway that goes through it.
Predjamski castle is near Postojna cave, roughly 45 minutes away on narrow country side roads. Unusual about the castle is that it is partially build inside a cave.
Koper is a lovely small Venetian town on the east Adriatic coast. It's known for the old and colourful buildings, narrow passages and streets, small cafes on well hidden between buildings town-squares and lot of artists and their workshops. Oh, it's also car-free :).

In the Istria region of Croatia you can go to Groznjan, Pula and Rovijn.
Groznjan has a wonderfully preserved stone houses and is the only municipality with absolute Italian minority in Croatia.
Main attraction of Pula is the Roman arena. Both Rovijn and Pula will expose to you their lovely Adriatic-Slavic spirit.

A convenient place to stay and explore both Istria and south Slovenia is the guest house of this guys in Momjan: https://www.airbnb.co.uk/users/show/4206263
We stayed there only for one night in 2014 but felt most welcomed. Marijana (owner's wife) even made pancakes with home-made jam in the morning.
Near their house is an amazing restaurant and its chef holds several prizes from culinary contests. Nearby is a fine winery, where we spent with joy a fair amount of money for tasty desert wines.
