Sales tax refund trouble in Czech Republic

Has anyone taken advantage of the sales tax refund in the Czech Republic?

In September we visited the Czech Republic and purchased some jewelry. When we arrived in Prague we went to the tax refund kiosk in Old Town Square. We presented our sales receipts and all of the information was then input into a computer. They also took our credit card information and were advised that we would need to mail or drop the original form in the box at the airport so that they would not take the refund back. We received the tax refund back in cash along with a form which we were instructed to drop in the special mail box at the airport when we left which was in 4 days.

We did as instructed, but in late December my credit card was charged back. Their claim is that we did not submit the form in the allotted 30 days.

Has anyone else had this problem?
