My husband and I booked a cruise leavening civitavecchia mid May. We added 5 and a half days in Rome prior to leaving on the sixth day to spend a day and an night in civitavecchia.
We need help deciding if we should book some tours in Rome now thru web sites and book transfers to get to civitavecchia from Rome and on the day the cruise returns book a transfer from civitavecchia to the airport or wait and do everything when we arrive in Rome

If you want to visit the Vatican Museums and the Colosseum, book those tickets in advance so you don't have to wait in the long lines to enter. I believe there are other places that you can book ahead of time, but I haven't visited those yet.

Reading your other posts, it looks like you are taking transfers both both trips to/from Civitavecchia. In which case, you should prebook Civitavecchia to FCO. Rome to Civitavecchia has other options and is less critical of the two.

Regarding "tour," it depends on where you are going and what you want to get out.

Colosseum underground tour (if offered at that time), Borghese Gallery (not tour, but reservation), Vatican Museum (reservation), any tour for Vatican need to be pre-booked. It is way too early to book, however.

The Vatican Museums and Colosseum only book a few months in advance if you are wanting to use tours from the official sites.

a favorite museum in rome is the villa borghese which you should book in advance... but really just a week or so. if you like guided activities, i can highly recommend cookingtoursinrome dot com which is just a several hour cooking class that ends with a wonderful lunch (that you just cooked.) the owner/chef is wonderful and patient and they have flawless feedback.

for the most part, you don't need to book tours far in advance except for those mentioned above, especially anything with the vatican and vatican museums.
