In Florence: Beccofino was fabulous (best meal I've ever had), but pretty expensive.
Il Latini is supposed to be great, but I didn't make it there. There was a another really good place where we were at a table next to Bobby Flay (from Food TV), but I can't remember the name offhand. I'll post back if I find it.

For Florence I recommend reading the website of Divina cucina. She has many reviews for the different areas in Florence. Every restaurant that we went to on her list was excellent - Gilda, Coquinarius and Cibreo Trattoria. Also wonderful was Trattoria 4 Leoni.

Florence: Osteria Del Agnolo on Borgo San Lorenzo between the Duomo and San Lorenzo Church.
Venice: Osteria Bancogiro near the Rialto Bridge. It's in a little piazza just as you arrive on the San Polo side of the bridge.

It's been posted here before, if memory serves (it's an NYT article by Apple on fresh fish in Venice).
I doubt that I can use any of the recs, but I'm bringing it with me just in case.
I canceled my reservation at Da Fiore and decided to eat near Padova. Da Fiore is the supposedly the best restaurant in Venice (even this one says so).

In Milan I would recommend da Puccini, Via S. Giovanni sul Muro, 5. We went be cause it was strongly recommended in Slowfood. It is not particularly fancy, but the food and wine list were superb. I would also recommend against the Trattoria Milanese, which was highly recommended in a number of travel books, but which I found to be touristy, overpriced and apparently living off its previous reputation.
