Passport application
 

My friend has agreed to consider international travel with me and is filling out the passport application for the first time. However, block 19 asks if he has ever been married and the questions in that section are not clear. What if he does not know his ex-wife's new married name? Does he just need to put in her maiden name? There is also a box for date of birth and for place of birth in section 19. I have to assume that those are for the ex-spouse since he already filled in that information for himself. But then there is the question that asks: Date of most recent marriage -- is that his or hers? what about the question that asks widowed? or divorced? Is that him or her?

I interpret it this way: the date of most recent marriage is his because he would fill this out only if HE had been married so they apparently want to know when HE was most recently married; date and place of birth are for the spouse (they want to verify in case he says the former spouse was/was not a citizen. The divorced/widowed is also about the applicant and not the former spouse.

It would help people answer if you said which country's passport application he is completing.

As a follow on...the divorced/widowed..if they want to find out this info about the former spouse they will get that information when/if the former spouse ever fills out a similar application...so it must be about the current applicant.

It is definitely a U.S. passport application...Form DS-11.
