What is the name of the cafe near Les Tuileries that is famous for hot chocolate?

Angelina. It's right on the Rue de Rivoli.

Rivoli is a long-g-g-g street so be sure to get off at the right spot. The number is 226 rue de Rivoli. Whenever I walked by night or late afternoon there were lines but I went late morning and it was half empty.
